The NHTD/TBI Waiver Service Coordinator is responsible for providing comprehensive case management services to clients, including needs assessment, service plan development, coordination of services, and monitoring of progress towards goals. The Service Coordinator works collaboratively with clients, service providers, and community resources to ensure that clients receive the services and support they need to achieve their goals.

Roles and Responsibilities

> Conduct comprehensive needs assessments with clients to identify their goals, strengths, and barriers to success. Minimum of 1 face-to-face home visit required.
> Develop individualized service plans that are client-centered, strength-based, and goal-oriented.
> Coordinate with service providers and community resources to ensure clients receive the services and support they need.
> Monitor client's progress towards their goals, adjust service plans as needed, and document progress in case files.
> Provide crisis intervention and support to clients as needed.
> Maintain accurate case records and documentation in accordance with agency and regulatory requirements.
> Participate in team meetings, case conferences, and trainings as required.
> Provide referrals to other services and resources as needed.
> Maintain confidentiality and ethical standards in all aspects of job performance.

BA required plus 3 years experience.

PBIS

We are looking for an experienced behavioral specialist to join our team. The successful candidate will be responsible for designing and delivering behavior modification programs to our clients, helping them develop positive behaviors to promote independent living and create systems to adjust to the community of their choice. The Positive Behavior Interventions Specialist will also be responsible for assessing the effectiveness of the training and revising goals. The ideal candidate will have excellent communication and organizational skills, as well as a passion for helping others reach their potential.

Responsibilities:

> Develop and implement training programs for clients with brain injury or chronic health conditions.
> Identify and assess training needs of clients.
> Conduct in home training.
> Evaluate the effectiveness of training programs and make necessary changes.

Must have:

- BA required plus 3 years experience

ILST

We are looking for an experienced Trainer to join our team. The successful candidate will be responsible for designing and delivering training programs to our clients, helping them develop life skills and create systems to live independently in the community of their choice. The Independent Living skills Trainer will also be responsible for assessing the effectiveness of the training and revising goals. The ideal candidate will have excellent communication and organizational skills, as well as a passion for helping others reach their potential.

CIC

We are looking for a Counselor to join our team and provide support to our clients. The ideal candidate will have experience working with a diverse range of clients, be able to provide a safe and supportive environment, and be able to develop individualized treatment plans that are tailored to each client's needs. The successful candidate will also have excellent communication skills, be able to work independently, and have a strong understanding of ethical and legal considerations.

Responsibilities:

> Establish and maintain a professional relationship with clients.
> Provide individual, family, and group counseling services.
> Assess clients' needs and develop individualized treatment plans.
> Conduct psychological assessments to diagnose mental health issues.
> Educate clients on mental health issues and available resources.
> Maintain accurate and up-to-date records of client progress.
> Work with other professionals to coordinate care for clients.
> Refer clients to appropriate community resources.

Must have:

1. Licensed Psychiatrist - Licensed by the NYS Education Department
2. Licensed Psychologist - Licensed by the NYS Education Department
3. Master of Social Work
4. Master of Psychology
5. Mental Health Practitioner - Licensed by the NYS Education Department
6. Certified Rehabilitation Counselor - Certified by the Commission on Rehabilitation Counselor Certification; or Certified Special Education Teacher - Certified by the NYS Education Department.

Each of these individuals must have, at a minimum, two years of experience providing adjustment related counseling to individuals and/or seniors with physical and/or cognitive disabilities and their families.
